Type 2 diabetes digital education and support. Online interactive self-management programme for people with type 2 diabetes.

Free Dental Helpline

Click here or call 0845 4647 for the NHS out of hours helpline and information. We cannot provide dental treatment or advice at Tywyn Health Centre.

 

Ear Syringing

We do not offer this service at Tywyn Health Centre but you can arrange an appointment at one of the private clinics mentioned above.

 

The Family Wellbeing Practitioner (FWP)

The Family Wellbeing Practitioner (FWP) team are part of a range of mental health and mental wellbeing services for children & young people under the age of 18 years old, providing early support & advice to young people and/or their parents to help address difficulties before it develop into more of a problem. The FWP can offer short-term support which may take the form of advice and signposting to other services in the community, support to access these services if required, and supported self-help to empower you to resolve your difficulties for yourself. Sometimes it can be helpful to talk with the FWP to better understand the issues and agree upon what might be most helpful in moving forwards.

If you are a young person who is worried about your mental wellbeing, or you are a parent/carer who is worried about a young person, please speak with your GP or Practice Nurse. They will decide what support might be most helpful. This might be to speak with the FWP (who works in the surgery as part of the primary care practice team) or it may be that you need more specialist support and they will be able to discuss these options with you.

 

Farming Community Network

Whether the issue is personal or business-related, FCN is there for you. We run a confidential, national helpline (03000111999) and e-helpline (help@fcn.org.uk) which is open every day of the year from 7am to 11pm. Their volunteers provide free, confidential, pastoral and practical support to anyone who seeks help.

 

South Gwynedd Foodbank

Mind

Nightingale House Hospice

Nightingale House Hospice run six-week Positive Action Programmes every month to educate & support people to self-manage their concerns & achieve improvement in their quality of life. Contact occupational therapist; Clare Williams on 01978 316800 for more information.

Parabl Talking Therapies

Click here to access Parabl Talking Therapies. They provide short-term therapeutic interventions for individuals facing common mental health difficulties or challenging life events which may be impacting on their emotional wellbeing.

 

Parenting Intervention to Prevent Affective Disorders in High-risk Adolescents Trial

Warwick University are running the PIPA (Parenting Intervention to Prevent Affective Disorders in High-risk Adolescents) trial, which is exploring the effectiveness of an online resource for parents/carers. They hope the resource will help reduce depression and anxiety in young people. 

 

PRINCIPLE Trial

Click here to join the PRINCIPLE Trial. Tywyn Health Centre is a member of this trail which aims to find low-risk treatments for older people with COVID. To be eligible you need to have experienced COVID symptoms for less than 15 days. The trial is open to people:

  • aged 65 and over.
  • 18-64 years & experiencing shortness of breath as part of COVID-19 illness.
  • 18-64 years with certain underlying health conditions.
 

Reading Well 

Click here to visit the Reading Well website. Reading Well supports you to understand and manage your health and wellbeing using helpful reading.

The books are all recommended by health experts, as well as people with lived experience of the conditions and topics covered and their relatives and carers.

You can be recommended a title by a health professional, or you can visit your local library and take a book out yourself.

There are five booklists available:

  • Reading Well for mental health
  • Reading Well for young people
  • Reading Well for children
  • Reading Well for dementia
  • Reading Well for long term conditions.
 

RCS Workforce

They offer one-to-one support (over the phone, Skype or Whatsapp) to keep you chipper. Free counselling or talking therapies over the phone

Samaritans

Save a Life Cymru

Click here to visit the website for Save a Life Cymru and learn CPR. Touch Someone’s Life, is a campaign, to encourage everybody in Wales to learn CPR skills so that they are confident of the steps they need to take when a person has a cardiac arrest.

 

SilverCloud

Click here to access the SilverCloud online course which offers support for anxiety, depression, and much more, all based on Cognitive Behavioural Therapy (CBT).

Tan Y Maen
